The Diagnostic Challenge: Biology More than Psychology
Bipolar Overview:
- Remember Bipolar is a *Mood Disorder,*
- ‘Not’ Primarily *Cognition* ‘Not’ Primarily *Impulsivity*
- But often does demonstrate Cognitive Changes
- Can occur with ADHD
Consider these Biologic Contributions: All can look ‘bipolar’ until corrected or directly addressed
- Sleep
- Stress
- Drug Reactions: Stimulants used with Depression
- Substance abuse
- Malnutrition
- Immune conditions: Gluten/Corn
- PTSD
- Estrogen Dominance
- Industrial toxins, paints
- Thyroid
- Brain Injury
Bipolar I: Are these symptoms Mania or Depression?
- >Shopping - Differential from Depression
- Manic Shopping Objective: Shopping for the neighborhood
- Art Buchwald in Martha’s Vineyard
- Depression Objective: Shopping to fill the hole in my heart
- >Sex – Differential from Depression Impulsivity becomes cyclic and repetitive
- Manic: I will make everyone happy they need me and will remember me
- Depressed: I need them, I don’t care what it takes.
- >Sleep – Impulsivity and sleep disturbance often connected to both Bipolar and ADHD
- Marked sleep disorder does not absolutely mean Bipolar
- Can be associated with ADHD
- >Stress - Cortisol – Adrenal dysfunction
Bipolar II: The Refractory Depression: Cormbidity the key – Some frequent examples
- Comorbid ADHD
- Drugs and ETOH
- Watch for *Clint Eastwood Depression:* More Explosive
- Sleep Deprivation and Paranoia
- Post Partum Depression – Estrogen Dominance
- Differentiate from ADHD, but May Include ADHD
ADHD Differential: Always Consider ADHD
- Predominantly cognitive
- ADHD More contextual [related to the reality], but can deteriorate with stress and sleep issues to non contextual
- Bipolar Moods more explosive and dangerous
- Both Can have Associated Sleep problems
- School History
- Bipolar is less contextual, more out of the blue
- Bipolar moods often last longer
- Bipolar less apologetic
- ADHD and Bipolar Both Often associated with TBI
